Answer - Most likely, outdated information about icon is stored in the browser cache, clean it.
Also, next trick will help: press left mouse button and move it without releasing. IE will reload the page, including the favicon!

Answer - It's an ordinary gif-file, and doesn't relate to favicon.ico,
it's recognized only by Firefox,
inserts into a page the same way, as favicon.ico:

Answer - Практически никаких специальных требований нет.
Следует иметь ввиду, что размер файла не должен превышать 8 мегабайт.
Также следует учесть, что все изображения приводятся к размеру 16х16 пикселей.

Answer - In case you have a possibility to create/edit ".htaccess" file in the root directory of your website, you should add line:
     AddType image/x-icon .ico
(Attention: filename ".htaccess" starts with a full stop symbol).

If you've got access to the server configuration file apache/conf/mime.types, add the following line:
     image/x-icon ico

Answer - Favicon is displayed browsers as an image 16x16 pixels.It's impossible to display an complex image in this way.
That's why there is only one rule of creating favicons - you should paint only one well-recognizable object with clean boundaries and a contrasting background.
Company Logo or as simple image are best suited for site icon
